WAIVER & POLICY INFORMATION

 

**Acknowledgment of Risk and Release of Liability**

 

In consideration of my child’s participation in the baseball tournament organized by PayDay Sports LLC (hereinafter referred to as "the Organizer"), I, the undersigned parent or legal guardian, acknowledge and agree to the following:

 

1. **Voluntary Participation**  

   I understand and agree that my child’s participation in the baseball tournament is entirely voluntary. I have reviewed all information provided by the Organizer regarding the event, including the inherent risks involved in playing baseball.

 

2. **Assumption of Risk**  

   I acknowledge that participation in baseball activities involves inherent risks, including but not limited to physical injury, illness, property damage, or other unforeseen events. I voluntarily assume all risks associated with my child’s participation in this event.

 

3. **Medical Authorization**  

   In the event of an injury or medical emergency, I authorize the Organizer, its staff, and designated representatives to seek medical treatment for my child. I agree to bear full responsibility for any costs incurred as a result of such treatment.

 

4. **Release of Liability**  

   I, on behalf of myself, my child, and our respective heirs, executors, administrators, and assigns, release and discharge PayDay Sports LLC, its officers, employees, agents, volunteers, and sponsors from any and all claims, liabilities, or demands arising out of my child’s participation in the baseball tournament, except where caused by gross negligence or willful misconduct.

 

5. **Photographic and Media Release**  

   I grant permission to the Organizer to photograph, film, or record my child during the event for promotional purposes. I understand that these images may be used in media, publications, and advertisements without compensation to me or my child.

 

6. **Code of Conduct**  

   I agree to ensure that my child abides by the rules and regulations set forth by the Organizer. I acknowledge that any disruptive or unsafe behavior may result in removal from the tournament without a refund of any fees paid.

 

7. **Indemnification**  

   I agree to indemnify and hold harmless PayDay Sports LLC and its affiliates from any claims, losses, or damages resulting from my child’s participation in the event.

I have read and fully understand this Waiver Form and agree to its terms. By signing below, I confirm my acceptance and acknowledgment of the risks involved in this activity and the conditions set forth by PayDay Sports LLC.

To ensure a safe, fun, and enjoyable experience for all participants and spectators, please adhere to the following housekeeping rules:

 

1. General Conduct

  • Respect all players, coaches, officials, and attendees at all times. Unsportsmanlike behavior, harassment, or disruptive actions will not be tolerated.

  • Follow instructions from tournament staff and facility personnel.

 

2. Facility Use

  • Help keep the facility clean by disposing of trash in designated bins.

  • No outside food or beverages are allowed unless approved in advance.

  • Smoking, vaping, and the use of tobacco products are strictly prohibited on the premises.

  • Alcohol is not permitted on-site.

 

3. Safety Guidelines

  • Always be aware of your surroundings, especially near batting cages and warm-up areas.

  • Parents/guardians are responsible for supervising children not participating in games.

  • First-aid assistance is available at the designated medical station. Notify staff immediately in case of an emergency.

 

4. Game Rules and Sportsmanship

  • Only registered players and approved coaches are allowed on the field and in dugouts.

  • Cheering and encouragement are welcome, but please refrain from negative comments or coaching from the stands.

  • Spectators must stay in designated viewing areas and off the field of play.

 

5. Parking and Transportation

  • Park only in designated areas to avoid fines or towing. Follow posted signs for overflow parking.

  • Do not block emergency lanes or other vehicles.

 

6. Prohibited Items

  • No weapons, fireworks, or other dangerous items are allowed on the premises.

  • Pets are not permitted, with the exception of service animals.

 

7. Lost and Found

  • Lost items can be reported or retrieved at the tournament’s information desk. PayDay Sports, LLC is not responsible for lost or stolen items.

 

8. Weather Policy

  • In the event of inclement weather, follow announcements and instructions from tournament staff regarding delays or cancellations.

 

9. Merchandise and Vendors

  • Support our vendors by visiting their booths. Merchandise sales are located at [designated location].

 

10. Photography and Media

  • By entering the facility, you consent to photography or videography for promotional purposes by PayDay Sports, LLC.
